I found a cloud labeled "fog/vapor cloud" (redirects to this page) on dungeon level 2 as a Monk, at the center of an otherwise empty 5x5 room on the far left. The door was locked, with just one entrance. Not sure what other generation details are relevant, but I notice that isn't included in the list of ways it can be generated.

The room also had inscribed on the square just past the door: "|f y?u war? to rcb ? ?hcp, train you? dog."
