When entering this level, there will probably be some flavor text about how you can see the walls of Dis towering above you, and above that, you can see the iron tower rising into the gloom above. The stairs will be on the left side of the map, in the maze. Demon gating is twice as powerful here (This comes from a line in the 3.5 Manual of the planes about how even small fights can become big)

One of the stairs up is a mimic. In the outer ring, there are some monsters with attack wands and breath weapons. Pack reflection! Instead of being placed in solid rock, the iron tower is placed in air

Dispater is placed on the Candelabrum of invocation, as well as his throne. Dispater is generated asleep.
